Donation

Our board is comprised of physicians in busy practices.  We seek to provide you and us with ease of donation, accountability, and flexibility.  We have established a relationship with Justgive, a San Francisco based 501-c3, which collects credit card donations made to them and directs the monies to us.  This organization charges three per cent as a credit card processing fee and sends a check to us monthly.  Their form 990s is available on line at their site. Please donate through them by visiting their website or using the donate button below.  You will receive a receipt for your tax free donation through them.

 In the future we plan to use GivenGain ,as well, as a donation management service.  They are located in the United Kingdom and have been supportive of Augustine in the past.  We will update this site when this relationship has been established.

Another option is to donate appreciated stock to American Friends.  This allows you to claim the appreciated value of the stock as a deduction on your taxes.  Consult your tax planner to see if this would be advantageous for you.

We have opened an account with Charles Schwab to serve this purpose.  If you will be contributing stock to our organization from your own Schwab account, download the following form from Schwab and follow the instructions there.  You will need to E mail us to find out our account number to complete this form.

If you are contributing stock from another firm (E*Trade, T. Rowe Price, etc.), then you will need to contact them about what forms are necessary.  You will need the DTC # for Schwab which is #0164.  You will also need to E mail us for our account information.

Finally, we strive to be accountable to you as a donor.  We will post our Form 990 annually so that you can review our tax returns. Our group will undergo an annual audit to insure accountability. Please review the documents section for more information.
